Annual Verification Forms

Click the link to complete the form. Once submitted, the form automatically submits a copy to Fusion Collegiate and will send you a copy as well. You do not need to email us the competed form. If you have multiple students attending Fusion Collegiate, a separate form will need to be completed for each child.

Demographic Verification Form – It is recommended that you use a computer for the demographic form as it is lengthy.

This form is an Alberta Education requirement as proof of Registration for funding purposes and includes your child’s personal information as well as parent / legal guardian and emergency contact information.

2025-2026 Authorization & Consent –  Combines the following forms:

-School District Use of Personal Information – explains how Fusion Education Association uses personal information in providing education services.

-Consent for FEA Use of Student Information – provides an option to grant / not grant consent for additional use of student information by school/district and third parties.

-Consent for Use of Student Information by News Media and Outside Groups (Third Party) – provides an option to grant / not grant consent for additional use of student information by outside groups and third parties

RELEASE OF INDEPENDENT STUDENT INFORMATON TO PARENT(S)/GUARDIAN(S)

Once a student reaches the age of 18, they are legally considered an adult, and under the Alberta Education Act – Student Records and the Freedom of Information and Privacy Act, Fusion Collegiate can no longer share your educational records, including grades, attendance, or other personal information, with parent(s) or guardian(s) without the student’s explicit consent.

It is important for students over the age of 18 to complete this form indicating their wishes to either continue providing access to their parents/guardians or not so that it is clearly stated for our records.
